Dear Parents,

Welcome to Memorial Elementary School.

I have provided some health information below, which I hope is informative. As always, please feel free to call my office at Ext. 2159 or email me at if I may be of any assistance answering your health concerns or questions.

If medication is essential during school hours, written orders from your physician on the Howell District Medication Form is required. This includes prescription and over the counter medication, such as Tylenol, Ibuprofen, Benadryl and cough drops. Please do not send cough drops to school with your child. Medications must be brought to school by the parent/guardian and given to the nurse. Forms may be obtained in the health office, or by following the link at the bottom of this information.

If you suspect your child is ill or not feeling well, inclusive of any: temperature of 100 degrees or above, persistent vomiting or diarrhea, frequent coughing or congestion, please keep them at home. Children should be fever-free for 24 hours, without the use of fever-reducing medication, before returning to school.

Daily absences and tardiness should be called to the school absentee line by dialing the main number at 732-919-1085 and choosing option #3. You may leave a message at any time throughout the day or night. Please indicate your child’s name, grade, and reason for absence.
Food Allergies
Many school children have food allergies which can cause reactions ranging from mild symptoms to severe and life-threatening breathing difficulties. The best treatment for any food allergy is PREVENTION. Please contact the school nurse if your child has a food allergy. This information should also be clearly indicated on the green emergency card for the nurse and the pink information card for the teacher.
Contact the Memorial Cafeteria staff to have the pertinent food allergy information entered into the food services computer.
Entry into 6th grade next September requires the administration of 2 additional vaccines: Tdap (Tetanus, diphtheria, acellular pertussis) and a Meningococcal vaccine. Please follow the link below for further information on this regulation and related forms.
